Here’s a case of one of our surgeons performing a combined scleral fixation of a subluxated IOL with an ERM peel. This patient presented with a symptomatic dense epiretinal membrane (ERM) and a subluxated IOL with the optic edge close the visual axis. Given the high risk of IOL dislocation, the patient was consented for possible IOL exchange just in case IOL respositioning was inadequate.
During the case, the subluxated sulcus IOL was initially repositioned however dislocation occurred towards the end of the vitrectomy and the decision was made to scleral fixate the existing IOL. The surgeon performed the Yamane Technique with modifications using a 27 gauge needle (rather than thin walled 30 gauge) and a bent 25 gauge end-grasping forcep.
