Following on from the previous video of the Four Flange Fixation with Canabrava technique, we demonstrate a couple of different ways to externalise the sutures.
In this case, we are already using 27g ports for a preceding 27g vitrectomy. The different techniques are:
1) Preloading the suture within 27g needle as described by @pedror.henriques
2) Docking the suture in the needle within the eye.
3) “Handshake” technique with 27g micrograspers through the superior 27g ports used for the vitrectomy.
Our preference is technique 2 as it is relatively easy to dock the suture within the needle’s lumen in the eye. A key step is to bevel the tip of the suture with a pair of scissors to allow much smoother docking.
